What is a primary care network (PCN)?
A PCN is groups of general practices working together with a range of local providers, including across primary care, community services, social care and the voluntary sector.
The aim is to offer more personalised, coordinated health and social care to their local populations.
A PCN is small enough to maintain the traditional strengths of general practice but at the same time large enough to provide resilience and support the development of integrated teams.
We have already worked together to share using the services of a clinical pharmacist, first contact physiotherapist and social prescriber.
We are also collaborating on a range of measures to share our ways of working and help continuously improve patient care and how we make the best use of our resources.
