You can contact us online about a non-urgent medical, administrative or prescription issue.
This short online form is the quickest and simplest way for the majority of our patients to access our team for assistance. Our practice phone lines remain open for anyone with accessibility needs who might find difficulty completing this form.
Please do not use this online form for urgent or emergency requests. The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday to Friday between the hours of 5.00pm and 9.00am. Please contact 111 during out of hours or 999 if this is an emergency.
NHS App
The NHS App lets you book GP appointments, order repeat prescriptions and access a range of other healthcare services.

The NHS App is for people aged 13 and over who are registered with a connected GP surgery.
With the NHS App you can:
- check your symptoms – find reliable NHS information on hundreds of conditions and treatments, and get immediate advice
- book appointments – search for, book and cancel appointments at your GP surgery
- order repeat prescriptions – see your available medications and request a new repeat prescription
- view your medical record – get secure access to your GP medical record
- register to be an organ donor – easily manage your preferences on the NHS Organ Donor Register
- choose how the NHS uses your data – register your decision on whether it can be used for research and planning

Patient Access
With Patient Access, you can now view, book and cancel appointments at the surgery from home, work or on the move — wherever you can connect to the internet. What is more, because Patient Access is a 24 hour online service you can do this in your own time, day or night.
Plus, Patient Access is not just for booking GP appointments, you can also:
- Order repeat prescriptions.
- Change your address details.
- Send secure messages to your practice.
- View your medical record securely online
Please note: your local practice may not offer every Patient Access feature. Ask at your surgery to find out which are available to you.
How do I register?
Bring one form of photo ID to the surgery and request a Patient Access Registration Form, we can then print out a form with an access code. Once you have the access code, you can register here.

All information that is sent to your surgery via Patient Access is secure. Your personal details are encrypted and protected using the highest standard internet security, so it cannot be intercepted. Only you and your GP surgery are able to see this information.
